Where it lives: Settings → Focus → pick a Focus (Do Not Disturb, Work, Sleep, etc.) → Focus Status → Share Focus Status.
What others see: in iMessage, a line like "[Name] has notifications silenced" with the option to break through. Apps Apple includes in Focus Status (Messages is the main one people notice) can show that state. Third-party apps only see it if they use Apple's Focus Status API and you allowed them.
Turn it off: same Focus Status screen, disable Share Focus Status for that Focus (or only allow certain people to know, depending on iOS version options). You can share status for Work but not for Sleep, for example.
Share Focus Status vs. Focus itself: Focus silences your lock screen and apps. Sharing only advertises that you're in that mode to people who message you. You can use Focus with sharing off and still get silence without anyone seeing a banner.
If you don't want Notify Anyway: that's the other person's choice on their device; you can't remotely block it. A tighter Focus that allows calls from nobody (or only Favorites) is how you reduce interruptions, which is separate from whether status is shared.
